Must have 16 CE Hours – certificates must be saved to upload. Do not attempt to upload certificates as separate files. *Upload one continuous file of certificates.
Evidence of $1,000,000 Professional and General Liability Insurance Coverage [Rule 532-X-3-.03(5)].
Renewal Fee $100 for two-year license. Process payment online.
Background check required. $25 fee paid separate from license fee and can only be done through this online application. No third-party background reports are accepted. Please check your email for the background check information.
Late fee $25 (expired less than one year), Reinstatement fee $75 (expired 1-5 years).
A massage therapist license expired more than five years may not be reinstated.
Licensed Massage Therapist (LMT)
Renewals 16 CEUs every two years
Initial 650 hours of education, passing the MBLEx or NCBTMB exam
